Perimenopause vs. menopause vs. post-menopause
Perimenopause begins 4–8 years before final menstrual period, typically in the mid-40s. This is when hormonal fluctuations cause irregular periods, hot flashes, and other symptoms. Menopause is defined as 12 consecutive months without a period — this is one point in time, not a phase. Post-menopause is everything after. The term 'going through menopause' usually refers to the perimenopause transition phase.
Factors that affect menopause timing
Earlier onset: smoking (−1–2 years), cancer chemotherapy/radiation (−2+ years), autoimmune conditions, lower BMI, nulliparity (no children). Later onset: higher BMI (through peripheral estrogen production in fat cells), more children, Asian ancestry. Genetics is the dominant factor — if your mother had early menopause, your risk is significantly elevated.
Surgical and premature menopause
Bilateral oophorectomy (removal of both ovaries) causes immediate surgical menopause regardless of age — this is qualitatively different from natural menopause due to the abrupt hormone drop. Premature ovarian insufficiency (POI) affects ~1% of women under 40. Hormone replacement therapy (HRT) is generally recommended for women who experience menopause before 45 to protect cardiovascular health and bone density.
